In case of a court decree/order concerning the status of a person, it shall be the duty of the cerk of court to advise

If it is another person who shall register the decree other than the clerk of court, the civil registrar shall verify if the
copy of the decision is authentic. Otherwise, he shall refuse the registration thereof. It is likewise the duty of the clerk of
court which issued the decree to ascertain whether the same has been registered, and if not, to have the said decree
recorded.
The civil registry ofice where the event of the decree/order was registered shall forward a certified true copy of the
decision to the Office of the Civil Registrar where an event afected was originally registered. The latter shall make the
proper annotations in the document and in the applicabe registry book. He shall likewise send a certified true copy of the
annotated document and the registered court decree to the Office of the Civil Registrar- General within ten (10) days

forwarded to the Office of the Civil Registrar-General
The supplemental report may be filed by the parent/ guardian or the party concerned, if of age, who shall

Filling an Application Form |
Plantilla personnel and casuals are entitiled to vacation, sick and special leave. Employees accrue leave credits (1.25 days each for vacation and
sick leave every month). Actual leaves are deducted from these leave credits. If an employee's leave period goes beyond the accrued leaves,
he/she will not be entitled to pay for the excess leave. Application leave must be filed at least 5 days before the leave. For sick leaves, the
