A. Crisis assistance

1. Intake of Client
CSWDO officer shall welcome clients (walk-in, referred, rescued, by court order or outreach) and conducts an initial interview.

  

2. Data Gathering
CSWDO shall further socio-economic data gathering through interviews, home visits, contact of families/relatives and collateral contact of friends/peers. The client’s medial information shall also be gathered (physical examination, dental check-up, psychiatric evaluation).

  

3. Formulation of Treatment Plan & Preparation of Case Study
The treatment plan and case study report shall be formulated by CSWDO through consultations with client and his/her family and staff involved, then a case conference with the client, his/her parents/guardian, teachers, psychologist/psychiatrist, lawyers, etc., shall ensue.

  

4. Implementation of Treatment Plan
Upon assessment of his/her needs, the treatment plan shall be applied and client shall receive the appropriate services and assistance for his/her case such as:
(1) residential care – shelter, food, clothing,
(2) Maternal & childcare – self-care of women, early childhood disorder, growth monitoring & immunization, nutrition
(3) livelihood skills development – leadership, workshop, sub-contract jobs,
(4) self-enhancement skills development – ego building, stress reduction/crisis management, values development,
(5) legal services – referral for lawyer’s assistance,
(6) educational services – formal/non-formal,
(7) transportation services,
(8) after care services,
(9) counseling, and
(10) medical services – referral for medical treatment/psychiatric evaluation.

  

5. Evaluation
Client’s case shall undergo a Pre-Termination Conference and feedback from field offices shall be collected.

  

6. Termination of Case
CSWDO shall prepare the closing summary for the termination of the case and the client can walk free to his/her new life. Or if CSWDO finds the client still in need of after care, the office shall prepare transfer summary and refer him/her to family & relatives or to institutions. Upon completion, the case shall be terminated.
