SECURING A NOTICE OF RETIREMENT FROM BUSINESS

Steps	Requirements	Fees	Person-In-Charge
*Business Owner or his/her authorized representative must bring current Mayor’s Permit to BPLO	Current Mayor's Permit Issued Business Plate Number		BPLO Staff
*Fill up the Notice of Retirement from Business.
*Proceed to City Treasurer’s Office who will sign the Notice.			City Treasurer
*Return to BPLO to surrender the Mayor’s Permit and Business Plate.
