A. Immunization Services
The City Health Office (CHO) provides immunization services for: (1) Tuberculosis, (2) Diphtheria, (3) Polio, (4) Measles, (5) Hepatitis B and (6) Tetanus Toxoid, for children from 0 to 11 months and pregnant women. This service is provided FREE OF CHARGE.
Frequency of Vaccination: a.) Polio – 3 x b.) Measles – once c.) Diphtheria – 3 x d.) Hepatitis B – 3 x
| Steps | Requirements | Person-In-Charge | 1. . Registration City Health officer or assigned nurse conducts the intake of personal data of the child or mother to be immunized.
