A. Immunization Services

The City Health Office (CHO) provides immunization services for: (1) Tuberculosis, (2) Diphtheria, (3) Polio, (4) Measles, (5) Hepatitis B and (6) Tetanus Toxoid, for children from 0 to 11 months and pregnant women. This service is provided FREE OF CHARGE.

Frequency of Vaccination:
a.) Polio – 3 x
b.) Measles – once
c.) Diphtheria – 3 x
d.) Hepatitis B – 3 x

1. . Registration
City Health officer or assigned nurse conducts the intake of personal data of the child or mother to be immunized. Then, checks patient’s immunization records.

2. . Immunization
Assigned health officer/nurse conducts the immunization of the child or mother.
 Health Officer / Nurse
3. Post Immunization Service
City Health Officer/assigned health officer/nurse gives the mother of the child/pregnant mother post immunization instructions and the schedule of the next immunization, as the case may be.
