F. Medical Certification

Firms and government agencies may require Medical Certificates from certain persons. This is especially true for those who are applying for a driver's license with the Land Transportation Office (LTO) and people seeking employment. Students are also required to secure a medical certificate upon before enrollment.

1. Payment of Certification Fee
Proceed to the City Treasurer’s office for payment of the Certification fee.
For applicants for a driver’s license:
a. Result of Drug Test
b. Certification fee
PhP 20.00
2. Client Registration
Present your Official Receipt to the City Health Office. City Health officer/assigned health staff shall accomplish the certification form and shall refer you to the Physician-on-duty.
 For employment and other purposes:
1.Results of
(a) Complete Blood Count/CBC,
(b) Urinalysis, and
(c) Chest X-ray
2. Certification fee
3. Assessment
Physician-on-duty shall assess and examine your medical condition, then signs the certification form.
 For students:
Certification fee
4. Issuance of Medical Certificate
The Certification shall be approved by the City Health Officer, then issue it to you.
